Mammography vs. breast ultrasound: what every woman should know about breast cancer screening

Mammography: the gold standard for breast cancer screening

A mammogram, particularly 3D mammography with tomosynthesis, remains the primary and most recommended screening tool for breast cancer. This is not up for debate: mammography is currently the only screening method proven to reduce breast cancer mortality.

Mammography detects approximately 4–6 cancers per 1,000 screenings and is highly effective at identifying early-stage breast cancers before symptoms appear, including calcifications that can indicate early changes in breast tissue.

Current guidelines recommend:

  • Annual mammograms starting at age 40 for average-risk women
  • Earlier or additional screening for women at higher risk
  • 3D mammography (tomosynthesis) when available, it improves cancer detection and reduces false positives compared to traditional 2D imaging

Mammography also has an important advantage no ultrasound can replicate: it is excellent at detecting calcifications, tiny calcium deposits that can be an early sign of breast cancer.

Dense breast tissue: why it changes the equation

  • Dense tissue appears white on a mammogram, the same color as tumors, which can make abnormalities harder to detect
  • Women with dense breasts have a slightly higher risk of developing breast cancer
  • Most states now require radiologists to notify women in writing if they have dense breast tissue after a mammogram

 

In women with dense breasts, adding ultrasound to mammography can detect approximately 2–3 additional cancers per 1,000 women screened, cancers that would not have been found by mammogram alone.

When is breast ultrasound recommended?

According to current NCCN guidelines, breast ultrasound may be considered as a supplemental screening tool for women with:

  • Dense breast tissue
  • Increased breast cancer risk
  • Inability to undergo MRI

 

It’s important to note that ultrasound is generally considered a third-line option for supplemental screening, after breast MRI and contrast-enhanced imaging. For women who truly need supplemental screening, breast MRI is significantly more sensitive, detecting approximately 13–23 additional cancers per 1,000 screenings compared to approximately 3 for ultrasound.

That said, MRI is not accessible or appropriate for every woman. Ultrasound remains a practical, non-invasive option when MRI is not available or suitable.
